Abstract AIM: To report one case of secondary keratoconus after LASIK and to discuss the prevention and treatment of the postoperative complication. METHODS: The cause and effective treatment of secondary keratoconus were analyzed by case report and review of literature. RESULTS: A 30-year-old female with no signs of early keratoconus preoperatively presented significant loss of visual acuity (VA) 7 years after LASIK. Corneal topography showed marked inferior protrusion in the left eye. The best corrected visual acuity (BCVA) of the left eye was 0.8 after rigid gas permeable contact lens (RGPCL) was used. CONCLUSION: Detailed preoperative screening, fine flap making and sufficient residual stromal bed thickness during operation, and timely follow-up are important for the prevention of keratoconus development. After diagnosis, proper management is essential to prevent progression, to promote visual rehabilitation, and to avoid corneal transplantation for the patients.
